We Partner with You to Create Health
In Depth Lifestyle / Integrative / Functional / Evaluation
We are qualified to practice functional medicine. Dr Bischoff is board certified in Internal and Integrative medicine and is also a certified Functional Medicine Practitioner.
The first step in the YBL approach involves filling out the intake form which can be downloaded on the website. Plan on spending at least 2 hours to fill this form out. Please bring this completed packet of information to your first appointment. It is best if it arrives at least 48 hours before your first appointment.
At the appointment be prepared to spend approximately 60-90 minutes with Dr Bischoff or one of our functional integrative dietitians. After careful assessment, a plan to create your optimal health will be developed with individualized recommendations for integrative lab testing and health optimization. If you are taking supplements please bring these to this appointment as well as a complete list of all medications. Some lab tests may be recommended to help us understand your individualized biochemistry better – these are not covered by insurance.
Creation of Your Personalized Health Plan
This will begin at the first visit with our clinicians. If Integrative lab testing is recommended it will be reviewed at the next visit to further identify long term goals and needs to create an optimal environment for your body to function.
Using a personalized plan, you will have the opportunity to begin implementing the recommended protocols. Follow up visits can be scheduled at timely intervals to assess progress and need for different interventions to achieve goals.
Foster health through valued relationships and patient feedback
We believe that the relationship of the clinician and the patient is primary to the healing process. We value your feedback so that we can create and sustain an environment that will help you achieve your personalized health goals and maintain optimal health.
